To request your medical records you can either use your online patient portal (MyChart) or submit your request in writing by mail or fax.
Please note: we cannot take verbal requests

MyChart, the patient portal, allows you to view, download and print your records online. Medical Records is open 8 a.m. to 4:30 p.m., Monday – Friday, excluding major holidays.
A top priority is to protect the security and privacy of patient information. All information contained within the patient medical record is considered confidential and is federally protected under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Medical records shall not be released without the written authorization of the patient, his or her legal representative, a subpoena or court order.
Go in person to the Arizona Office of Vital Records or a county health department's Office of Vital RecordsFill out the state's application and mail it in at the address provided on the form, along with payment and items in the Customer Mail In Checklist on the form.Order online at the Arizona Office of Vital Records.
- Married or if not adding a father: Please wait seven business days from birth before purchasing.
- Unmarried with signed "Acknowledgement of Paternity" paperwork: Please wait 21 business days from birth before purchasing.
- Unmarried couples: If you did not complete "Acknowledgment of Paternity" paperwork in the hospital before discharge, you will need to call the Office of Vital Records to make an appointment. You will need to bring a government-issued ID to that appointment.
Your baby's Social Security card should arrive to the mailing address you provided within three to 10 weeks. If you do not receive the card within this timeframe, please call Social Security Administration, (800) 772-1213.Important note: If you do not have a name for your baby, it is not possible to issue a Social Security card.
